National Health Care Decisions Day is an excellent reminder to plan for end-of-life decisions. These decisions about the health care you want to receive in the future are not easy, but Consider The Conversation can help you work through many of the items to consider.  You direct your health care wishes in advance of when you may not be able to through an Advance Health Care Directive. These documents vary by state, but the Advance Health Care Directive link (highlighted above) from the Expert Help tab of PersonCare.net gives plenty of information for you to get started.

Take the time now while you are still able. Any person over the age of 18, who understands what they are doing, should have an Advance Health Care Directive. Do leave something for your loved ones. Not just money and stuff, but an Advance Health Care Directive.
